To ensure a smooth and efficient first visit at Lake Nona Chiropractic, we ask all new patients to complete a set of forms before their appointment. This helps us understand your health history and current needs, allowing us to provide the best care possible. It is important to note that you MUST print out or email us the completed forms.

Here's What You Need to Know:

• Complete Before Your Visit: Please fill out the required forms prior to your first appointment. This will save you time and help us get you started on your path to wellness as soon as you arrive.
• Date Your Paperwork: Ensure all forms are dated the day of your first appointment.
• 'Not Applicable' Items: If a section doesn't apply to you, simply mark it with “N/A.”
• Check All Boxes: Please make sure to check each box individually to confirm all items are covered.
• Answer Every Question: Each question must be answered before your appointment to provide a complete understanding of your health background.
• Auto Accident Patients: If your visit is related to an auto accident, please complete the New Patient Paperwork and the PIP Paperwork.
• Bring Forms to Your Appointment: Don't forget to bring your completed forms with you.
